VIM Workflow error - Cannot initialize the Approval Process - check configuration

Hi,

I get the following message when someone is trying to approve a workflow:

The DP document dashboard shows there is no Requisitioner Name and email fields are blank.  Can anyone advise how to correct the issue and which part of the configuration is missing?
