When we completing work flow it was completing successfully, but new workflow(Work item) is creating automatically under the same reference no of T5asrprocesses table,with same data.

When we initiating and approving the work flow (Work Item) it is completing fine. But new WI is creating with same data. This issue happening for few work items(WF processes) only, not for all the work items .

I am new to the work flow. Can you please any one suggest us ,  What are the things we need to check ?

And also please let us know, how the work flow processes will create the new WI when it is initiated and approved from portal?

Please help us to resolve this issue.
