Workflow doesnt Trigger sometimes quality Notification

Hello Experts,

We are getting a very surprising issue wherein workflow doesnt trigger sometimes. workflow should trigger whenever any quality notification is created i.e BUS2078 and event CREATED.

Now, when Notification of Type WD is created then we have different process of sending emails through BRF+. What is happening is user created 3 quality Notifications of Type WD on production and for one Notification, workflow was kicked off and emails were sent through BRF+ but for the other 2 notifications ,workflow was not kicked off.

Please help what could be the possible reason of the same.
