Workflow Completed but table T5ASRSTEPS not updated

I am facing issue where the workflow for the form is completed but the table T5ASRSTEPS is not updated as 'Complerted'. Can someone please tell me where is the table T5ASRSTEPS updated from and what are the possiblities which may cause this discrepancy? The issue is not occuring for all the forms but only for some of the forms.
