problem with mail sending report RSWUWFML2

I am sending mail using report RSWUWFML2, it is working fine up to final level approver. But in final level the mail is triggering 4 times for the same user. There is no substitute maintained .

Any one can help on this issue. Please find the below SOST screen shot.
