How to add Comments in the Decision step

Hi,

I got the requirement that i need to add a Comments in the Decision step. When the supervisor gets the approve workitem.

For Example:

In the Decision step supervisor can approve or reject the request.

In that he can write the Reason for rejection and reson for Approve.

How to do it?????
