Workflow completed but no PO has created

Hi Experts,

I have got an issue with my workflow,

issue is whenever a PR has been created workflow will trigger and after releasing that by the agent it it will create the PO.

I did not see any issue till date since 1 year, recently I found this issue to one PR when I check the workflow status it is in completed status.

Please guide me how to fix this issue.
