After final approval of Document, it is not showing in workflow inbox of initiator as Approved

Hi,

I am facing few  issues in production as I am  new to workflow your help will be very much appreciated.

The problem are:

1.)After final approval of Document, it is not showing in workflow inbox of initiator as Approved.Earlier it was working fine.

2.)After Final Approval of Document, If the user was in Offline then it will shows as “you have unread documents in your inbox”. And if user was in online then it will give a popup like “PO number XXXXX is Approved”. Now Pop like “PO number XXXXXXX is Approved” is not coming on PRD.

Kindly help me.
