Update Termination error when calling workflow from IW31/IW32

Dear Experts,

I was working on a development to trigger a workflow whenever a maintenance order has been created / changed using transaction IW31/IW32.

Using the below user-exit i was triggering the workflow.

IWO10009- PM Order: Customer Check for 'Save' Event - EXIT_SAPLCOIH_009

But whenever we trying to create a maintenance order am getting the below update terminated error.

Internal session terminated with a runtime error RAISE_EXCEPTION (see ST22)

Exception condition "DUMP_STATUS_INCONSYSTENCY_ERO" triggered and not allowing me to release the maintenance order.

please help me to resolve this issue.
