Workitem is not reflecting in UWL

Hi ,

I'm currently working on workflows for process and forms. The workflow is getting triggered, workitem is getting created but the workitem goes directly into "Not reserved by agent" status in SWI5. The workflow log shows the correct agent name and user id. As the Workitem goes to "Not reserved by agent status", the workitem is not appearing in UWL.

Master data is also maintained correctly, user validity also exists.

Any suggestions why the workitem is going to that status, also any leads from where to start debugging would be helpful.
